THE HOUSING CHOICE SECTION 8 VOUCHER PROGRAM is designed
to allow people who have very low incomes the chance to rent decent,
safe, and sanitary housing at a rent they can afford.
SCHA provides participants looking for a place to live with a list of
landlords willing to participate in the Section 8 program. If you would like
to be on this list, please email your name, size of unit, county, and phone

Section 8 Landlord Information
Request for Tenancy Approval
This is the first form we need completed by a landlord in order to calculate
gross rent.
Tenancy Addendum
Needs to be attached to the lease
Landlord Certification
To be completed by landlord before signing a contract with SCHA.
Lead-based Paint Disclosure
Complete if home is built prior to 1978.
W9
To be completed before any payments can be paid to the landlord.
Direct Deposit Corporation Individual
Request for Rent Increase
Complete and submit 60 days before effective date of rent change.
Mutual Rescission of Lease
Complete if landlord and tenant agree to terminate lease early.
